Kinship Caregiver Forms

The Colorado Division of Child Welfare provides the following forms to help county departments connect with, evaluate and support kinship caregivers when a child enters the child welfare system. These tools facilitate stable, familiar placements for children by streamlining the communication and certification process.

Notification to Potential Relatives and Kin

When a child is temporarily removed from their home, caseworkers send this informational packet to extended family and trusted adults. It explains how relatives can support the child, such as providing temporary placement, supervising family visits or assisting with transportation.

The packet outlines:

• Caregiver rights and responsibilities.
• Available financial resources.
• Required safety background checks.
• A Kinship Identification Form to help caregivers list other potential family members or supportive adults.

Kinship Foster Care Training and Support Verification Form

Kinship caregivers must complete six hours of training on topics such as parenting, family dynamics and trauma-informed care to qualify for foster care certification or recertification.

This form helps applicants and agency workers:

• Record the training date and format (in-person class, video, or book).
• Document total hours completed.
• Describe how the caregiver will practically apply this information to support the children in their care.
