Privacy Statement

Thank you for visiting the Colorado Department of Human Services’ (CDHS) Privacy Statement webpage and reviewing our Privacy Statement for the MyCOBenefits mobile application. This statement addresses collection, use of, and access to information that may be obtained through use of the MyCOBenefits mobile app. Download the statement (Google Doc). Information Collected

“Personal Information” is information about a natural person that is readily identifiable to that specific individual. Personal information includes such things as an individual’s name, address, phone number, or email address.

We collect no personal information about you unless you voluntarily participate in an activity that asks for information. If you choose not to participate in these activities, your choice will not affect your ability to use any other features of our MyCOBeneftis mobile application.

If personal information is requested on the MyCOBenefits mobile application or volunteered by the user, state and federal law may protect it. However, this information is a public record once you provide it, and may be subject to public inspection and copying if not protected by federal or state law.

Users are cautioned that the collection of personal information requested from or volunteered by children on-line or by email will be treated the same as information given by an adult, and may be subject to public access.

In addition to Personal Information, information accessed or transmitted through the MyCOBenefits mobile application may include benefit identification numbers, eligibility and coverage information, health plan enrollment information, and other information necessary to support program services. The MyCOBenefits mobile application itself does not independently maintain a separate database of benefit eligibility or health information.

Furthermore, the information contained in many kinds of documents filed with the State of Colorado is electronically stored outside of the MyCOBenefits mobile application and may be made accessible to the public through the State of Colorado’s website.

We may collect some information about your visit to the MyCOBenefits mobile application. This information may include:

  1. The Internet Protocol Address and domain name used, but not the email address. The Internet Protocol Address is a numerical identifier assigned either to your Internet Service Provider, your device, or to your computer. We use the Internet Protocol Address to direct Internet traffic to you.
  2. The type of operating system or browser you used.
  3. The date and time that you visited this site.
  4. The web pages or services you accessed through the MyCOBenefits mobile application.

The information we collect or store is used to improve the content of our MyCOBenefits mobile application and to help us understand how people are using our services.

If during your visit to our MyCOBenefits mobile application, you send an email to us, the following information will be collected:

  1. The email address you used in sending the email.
  2. The contents of the email.

We use your email to respond appropriately. This may be to respond to you, to address issues you identify, to further improve our MyCOBenefits mobile application, or to forward the email to another agency for appropriate action.

Cookies

To better serve our users, we may use “cookies” to customize your electronic experience with us. Cookies are simple text files stored on your device by your web browser or MyCOBenefits mobile application. They provide a method of distinguishing among visitors to our MyCOBenefits mobile application.

Cookies created on your devices by using our MyCOBenefits mobile application do not contain personally identifying information and do not compromise your privacy or security. We use the cookie feature only to store a randomly generated identifying tag on your devices.

You can refuse the cookie or delete the cookie file from your device by using any of the widely available methods. Our services are provided in such a way that the use of cookies is not mandatory.

How Data is Deleted

Users may remove the MyCOBenefits mobile application from their mobile device or computer at any time. Removing or uninstalling the application does not delete information stored in the Department’s eligibility and enrollment program systems. Because information entered through the mobile application becomes part of official program records, it cannot be deleted solely by removing the mobile application.

Members who wish to update or correct information associated with their MyCOBenefits mobile application may do so through the Health First Colorado mobile application, their Colorado PEAK account, or by contacting their county department of human or social services.

Requests will be reviewed and processed in accordance with federal and state program requirements. Some information must be retained for the minimum record-retention period required by federal regulations and State of Colorado records management policies.
